If you have a chronic medical condition, you would be eligible for medicare funding for Care Plan. This is a referral to see any allied health professional and medicare will fund for up to 5 visitis a year to help manage your medical condition (up to a certain amount). We will usually refer to a bulk billing allied health professional. For example if you have diabetes you will need yearly foot checks by a podiatrist. Please let us know so that our doctors can draw up a Care Plan for you. Care plans are reviewed on a regular basis so our doctors may recall for reviews.

If you are suffering from a mental health condition, you will be eligible to medicare funding for mental health care plan. This is a referral to see any psychologist and medicare will fund up to 10 visits a year (up to a certain amount). Please let us know so that our doctors can draw up a mental health care plans for you. The plan is reviewed on a regular basis.

If you are between age 45 to 49 or age over 75, you will eligible to health assessments. This is a general overview of your own health and it usually involve having blood tests - for example checking cholesterol.

We use National Home Doctor Service for after hours home visits.
A home visit doctor can be arranged in the after hours period for any medical issue.
Once home visits are done, a report will be sent to us for continuity of care during the day.
